When we're talking about ages for characters in an RP, do we use actual years (such as a lion being an adult at 3) or do we use human lifespan years (such as a lion being an adult at 21)? It's lead to some confusion when I told people that my fully adult honey badger character is 4 years old.

There's no official way to do it - some people use actual years and some use human years. You just have to specify in the character's profile what kind of "years" you're talking about.
It might be easier to use terms like "adolescent" or "young adult." This is unambiguous and immediately communicates the approximate age range of your character. Human years are useful for the same reason, as long as you specify they're in human terms. I think a literal age is probably the most ambiguous method - since I don't know the typical lifespan of a honey badger, I don't know what it means for a honey badger to be four years old.

Generally we've been using approximate aging such as cub, adolescent, young adult, adult and elder. I recall seeing some characters with human equivalent ages, or the actual years in the profiles, but right now I can't remember which characters those might have been. My personal opinion/preference is using the approximate aging as it's easier to grasp.
